"You" refers to the LTCF or individual who agreed/signed to this document.
"LTCF Users" refers to the client who is using any CCMS applications such as “CCMS Activities.”
"CCMS-eMAR" refers to "Electronic Medication Administration Record", one of Our company's products and services. The service is delivered through CAREMONITORINGSYTEM.COM
"CCMS-Activities" refers to one of the products and services offered by Our Company, which tracks all of the Resident's major activity records and behavior logs. This service is provided through CCMSACTIVITIES.COM.
The Website is offered and available to Users who are a minimum of 18 years old. You should not use the Website if You are younger than the aforementioned age. By registering and creating an account, You represent and warrant that You are legally permitted to enter into a binding contract, and You meet the preceding conditions concerning the User's eligibility.
The Login credentials shared with You or a particular account shall not be shared with or used by a third-party. You understand that the credentials that We provide are only meant to be used for logging in by a single user or the person who has signed up for the Services.
In order to use our services, your administrative systems and procedures, along with your staff's training protocols, should all be HIPPA compliant. Safeguarding Your account is a crucial function of HIPPA compliance, which is why You must protect yours and your Resident’s data with a strong password when accessing the Services vis-à-vis the Website. To prevent any fraudulent activity and to verify Your identity at the time of logging on the Website, We provide you with two factor authentication each time you change networks, computers, or attempt to login on any new device. However, You are liable for safeguarding the activities and constituents associated with Your account.
LTCF creates its password and account security measures for their internal employees or LTCF Users. We are not responsible or liable in any manner for passwords or security measures established by LTCF. LTCF is exclusively liable for the password and security measures it provides for its Providers’ and its Residents data.
When accessing or using CCMS, LTCF saves client’s data on the CCMS database. We deploy advanced technology for safeguarding CCMS database however, We will not be responsible for any data theft or unauthorized access to the database arising out of LTCF's client or LTCF's negligence concerning the handling or managing of their respective passwords or failing to observe security protocols.
To confirm Your identity, We may ask identity verification and accounts protection questions, distinctive ID number and other similar measures to establish Your identity.
The content You post, transmit or share on the Website are exclusively Yours. We are not liable or accountable for the content You post, share or transmit on the Website.
If there occurs an activity associated with Your account, that causes any fraud, loss, damage or similar consequences and such activity occurs because of Your negligence in keeping Your login credentials in order or safeguarding Your account. CCMS is strictly a private Website for individual LTCF, and We do not embed any third-party links or advertisement links in the CCMS Website, unless we determine it as beneficial for Us and for providing Your service.
We may disclose information to law enforcement agencies, or under a direction by court of law or to cooperate with investigating agencies concerning any investigation. We shall only disclose after examining the bona fides of such request made to us. Information such as name, email address, contact number, city, state, shall be disclosed to the law enforcement agencies under such a request.
We shall also disclose information to law enforcement agencies or similar regulatory bodies if we believe in good faith that it is absolutely necessary to prevent You from imminent harm (physical or financial) or occurrence of any illegal activity.

You agree not to use CCMS for spamming or automated mailings that overloads Our server or access the Website for asking other users or competitors to join any scheme or program.
You agree not to use CCMS for breaching other user's account or steal data from their account. Besides, You shall not use Our network or system to gain access surreptitiously to a device, another account, service or data.You agree not to misrepresent, conceal or claim to be an impostor when accessing our Services. You shall not represent yourself to be associated with any organization or device by using a fraudulent ID.
Our team may communicate with You on the number provided by You when signing up on the Website.
